  1. Mail a check for $20 to the Foundation office: PO Box 8066, Dothan, AL 36304 (if you are reserving multiple spots, please update the total: $40 for 2 reservations, $60 for 3 reservations, etc.)
  2. In the memo line, include the event location you are attending (Crestview, FL or Montgomery, AL) and who the reservation(s) are for.
  3. Send an email to [email protected] so we know your reservation is on the way!
